About the role
As a Home Health RN, you will be a clinical leader applying your expertise directly in a patient's most comfortable environment to deliver high-quality, personalized nursing care. You will craft individualized care plans, collaborate with a multidisciplinary team, empower and educate patients and families, mentor and guide LPNs, LVNs, and aides, and innovate with technology to optimize care and documentation.

Responsibilities

- Collaborate and coordinate. Work seamlessly with a multidisciplinary team – physicians, therapists, and support staff – to ensure holistic care.
- Empower and educate. Serve as a vital resource for patients and their families, providing crucial teaching and emotional support.
- Mentor and guide. Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team.
- Innovate with technology. Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation.
Qualifications
- Graduate of an accredited nursing program with an active RN license in the state served.
- Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
- Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
- Current CPR/BLS certification.
- Valid driver’s license and reliable transportation.

Pay
$35.53 - $52.85 /hour
