About the Role

As a Home Health RN, you will deliver high-quality, personalized nursing care directly in patients' homes, fostering independence and well-being in a comfortable environment.

Responsibilities

• Act as a clinical leader, crafting and dynamically adjusting comprehensive, individualized care plans.
• Collaborate with a multidisciplinary team—physicians, therapists, and support staff—to ensure holistic care.
• Empower and educate patients and their families, providing teaching and emotional support.
• Mentor and guide LPNs, LVNs, and Home Health Aides to build a cohesive care team.
• Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation.
• Benefit from a 1:1 patient-clinician ratio, allowing undivided time for assessments, education, and compassionate care.
• Guide patients through their full healing process, from initial visit to recovery, adapting care plans and celebrating milestones.

Requirements

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a plus).
• Exceptional critical thinking, clinical expertise, strong case management abilities, and a commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Pay

$35.53 - $52.85 per hour.
