About the Role
As a Home Health RN, you will deliver high-quality, personalized nursing care directly in patients' homes, fostering independence and well-being in a comfortable environment.
Responsibilities
- Act as a clinical leader, crafting and dynamically adjusting comprehensive, individualized care plans.
- Collaborate with a multidisciplinary team—physicians, therapists, and support staff—to ensure holistic care.
- Empower and educate patients and their families, providing teaching and emotional support.
- Mentor and guide LPNs, LVNs, and Home Health Aides to build a cohesive care team.
- Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation.
- Benefit from a 1:1 patient-clinician ratio, allowing undivided time for assessments, education, and compassionate care.
- Guide patients through their full healing process, from initial visit to recovery, adapting care plans and celebrating milestones.
Requirements
- Graduate of an accredited nursing program with an active RN license in the state served.
- Minimum of 1 year of nursing experience (Home Health experience is a plus).
- Exceptional critical thinking, clinical expertise, strong case management abilities, and a commitment to patient-centered care.
- Current CPR/BLS certification.
- Valid driver’s license and reliable transportation.

Pay
$35.53 - $52.85 per hour.
