Responsibilities
- Act as a clinical leader, delivering high-quality, personalized nursing care in patients' homes.
- Develop, implement, and adjust comprehensive care plans to promote patient independence and well-being.
- Collaborate with a multidisciplinary team—physicians, therapists, and support staff—to ensure holistic care.
- Empower and educate patients and their families, providing teaching and emotional support.
- Mentor and guide LPNs, LVNs, and Home Health Aides to foster a cohesive care team.
- Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation.
Requirements
- Graduate of an accredited nursing program with an active RN license in the state served.
- Minimum of 1 year of nursing experience (Home Health experience is a plus).
- Exceptional critical thinking, clinical expertise, and case management abilities.
- Unwavering commitment to patient-centered care.
- Current CPR/BLS certification.
- Valid driver’s license and reliable transportation.
