Responsibilities

• Act as a clinical leader, delivering high-quality, personalized nursing care in patients' homes.
• Develop, implement, and adjust comprehensive care plans to promote patient independence and well-being.
• Collaborate with a multidisciplinary team—physicians, therapists, and support staff—to ensure holistic care.
• Empower and educate patients and their families, providing teaching and emotional support.
• Mentor and guide LPNs, LVNs, and Home Health Aides to foster a cohesive care team.
• Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation.

Requirements

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a plus).
• Exceptional critical thinking, clinical expertise, and case management abilities.
• Unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Pay

$35.53 - $52.85/hour
