Responsibilities

  • Provide professional nursing care to patients in their home after normal business hours or weekends.
  • Achieve compliance with physician orders and maintain visit frequency as per orders.
  • Document care plans and interventions accurately and in accordance with policies and procedures.
  • Care holistically for patients and their significant others, ensuring a supportive environment.
  • Ensure patient safety and competence in performing assigned treatments and procedures.
  • Collaborate with interdisciplinary teams and document all activities appropriately.
  • Stay compliant with payor sources and documentation guidelines.
  • Meet productivity and documentation timelines, updating care plans as needed.
  • Teach patients and significant others according to the plan of care.
  • Report and address patient care issues promptly and effectively.
  • Coordinate services with other healthcare professionals and manage shifts efficiently.
  • Communicate patient information and care plans during transfer of care.
  • Perform supervisory duties, including completing visits and evaluating staff performance.

Qualifications

  • Associate's Degree in Nursing or Diploma Certificate in Nursing.
  • Minimum of six (6) months of RN experience or one (1) year of LPN experience, preferably in home care or hospice.
  • Excellent written and verbal communication skills.
  • Proficient in computer skills, including Microsoft Office and/or Google Suite platforms.

Skills and Abilities

  • Knowledge of home care and hospice practices.
  • Ability to lift a minimum of 50 lbs., push/pull a minimum of 50 lbs., and stand for 8 hours a day.

Pay

The starting rate of pay is $90,272.00 annually. Actual compensation may vary based on various factors including labor market data, years of relevant experience, education, certifications, and geographic location.
