About the role
Provides in-home clinical nursing care consistent with professional standards for patients in the UNC Health Advanced Care at Home Program. Reporting and accountable to the Nurse Manager/Director, hospital level care to the patient utilizing established nursing processes in a home setting.
Responsibilities
- Participates in identifying and meeting learning needs of self.
- Attends education programs based on identified learning needs.
- Uses patient education materials relevant to patient population.
- Assesses readiness to learn of the patient/family/caregivers.
- Assesses developmental level of patient and factors affecting ability to learn.
- Evaluates care with guidance as needed.
- Communicates relevant information to promote continuity of care.
- Demonstrates competence in care of patients with complex problems.
- Performs in-home, bedside lab draws/point of care testing as required.
- Utilizes healthcare organization and nursing standards, policies and procedures in delivery of care.
- Organizes and prioritizes care according to patient/family needs.
- Consults with appropriate resources for complex care issues.
- Develops and achieves goals related to professional growth.
- Collaborates with patient and family/home caregiver to prepare/update the plan of care.
- Coordinates with Command Center team regarding courier services.
- Reads journals applicable to practice.
- Bridges gaps between staff for assessment of applicability.
Requirements
- Graduation from a school of professional nursing.
- Licensed to practice as a Registered Nurse in the state of North Carolina.
- Basic Life Support (BLS) for Healthcare Provider certification.
Qualifications
- Two (2) years of nursing experience.
Skills
- Knowledge of healthcare organization and nursing standards, policies and procedures.
- Ability to prioritize tasks for patients with complex problems.
- Effective communication skills.
- Ability to work collaboratively with interdisciplinary team.
- Ability to assess and anticipate discharge needs.
- Understanding of social determinants of health.
