About the role
The RN Clinical Care Coordinator is responsible for managing a panel of members with complex medical and behavioral needs, providing care coordination to support medical, behavioral, and socioeconomic requirements.
Responsibilities
- Conducting comprehensive needs assessments
- Developing and implementing person-centered care plans
- Collaborating with care teams and community resources
- Providing member education
- Supporting discharge planning
- Advocating for members' needs
Qualifications
- Current RN license in MA
- At least 2 years of clinical experience
- Proficiency with MS Office
- Reliable transportation
Preferred qualifications
- Bachelor's or Master's in Nursing
- CCM certification
- Experience in community case management or managed care
Physical requirements
Travel within the territory and ability to navigate various locations.
