RN Field Case Manager- Orland Park, Tinley Park & Surrounding Areas

About the role

Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois, providing serious illness care, hospice care, and grief support for adults and children. The Field Registered Nurse (RN) Case Manager is responsible, as a member of the interdisciplinary team, for the provision of skilled nursing assessment and hospice care to maximize the comfort and health of patients and families/caregivers consistent with Lightways policies and procedures. The RN plans and directs services to meet the needs of individuals and families/caregivers within their homes and communities.

Responsibilities

• Deliver skilled nursing care and hospice services in home and community settings
• Conduct comprehensive patient assessments and develop individualized care plans
• Administer medications and treatments while monitoring patient symptoms and outcomes
• Educate patients and caregivers on care, self-management, and disease progression
• Collaborate with the interdisciplinary team to ensure coordinated, high-quality care
• Maintain accurate documentation and participate in on-call coverage as needed

Schedule & territory

• Cover DuPage and Southern Cook County (Tinley Park, Orland Park, Downers Grove, Lemont)
• Monday–Friday schedule
• Minimum of 20 on-call hours per month (may increase), supporting entire service area

Qualifications

• Current RN license
• Graduate of accredited nursing program
• Minimum of 2 years of nursing experience, with at least 1 year in public health, home care, or hospice preferred
• Valid driver's license and insured, reliable vehicle
