RN Field Case Manager

About the Role

We are seeking a registered nurse to provide in-home assessments, patient education, and care coordination in a community-based setting. Responsibilities include evaluating members' health, identifying treatment barriers, making referrals, and supporting self-management. Maintain communication with members through in-person visits or phone interactions to ensure quality care continuity.

Responsibilities

• Maintain communication with members via in-person visits or phone interactions
• Ensure continuity of quality care

Requirements

• Unrestricted RN license in Washington
• Basic Life Support certification or ability to obtain
• At least 1 year of clinical and home care experience
• Valid driver’s license and reliable transportation

Preferred Qualifications

• BSN or CCM certification
• Experience in discharge planning, utilization review, or risk management
• Telephonic care experience
• Bilingual in English and Spanish
• Proficiency in MS Office
• Managed care background
