About the role
The position involves providing skilled nursing care to patients in their homes, following physician orders and the plan of care under the direction of the Clinical Manager/Nursing Supervisor.
Responsibilities
- Administers services in accordance with the plan of care.
- Makes initial and regular evaluations of the patient's nursing needs.
- Initiates and revises the plan of care.
- Provides specialized nursing services and preventive/rehabilitative procedures.
- Prepares clinical and progress notes and summaries of care conferences.
- Collaborates with other healthcare professionals and educates patients and families.
- Participates in in-service programs and peer review.
- Completes certifications, discharge summaries, and OASIS assessments.
- Utilizes ICD-10 codes appropriately.
- Supervises and teaches other nursing personnel.
- Conducts patient care conferences and participates in utilization reviews.
- Takes on-call duty as assigned.
Qualifications
- Graduate of an approved school of professional nursing and licensed in Michigan.
- One year of nursing experience.
- Kinnser - EMR experience preferred.
- Home Healthcare experience preferred.
- Experience with wound vac, PICC line, and mediport care preferred.
- Pediatric experience desired.
