About the role

The Diabetes Educator at Sanford Health's North Peds Clinic is involved in assessing, diagnosing, educating, and managing patients' diabetes conditions. This role includes providing one-on-one and group education sessions, developing personalized care plans, monitoring patient progress, and collaborating with other healthcare professionals.

Responsibilities

  • Provide one-on-one and group education sessions on diabetes self-care.
  • Develop and implement personalized care plans for patients.
  • Maintain and monitor patient health status through regular data collection.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.
  • Stay updated on the latest diabetes management techniques and participate in community outreach.
  • Manage 7th weekend call rotations at the Medical Center.

Requirements

  • Must have a bachelor’s degree in nursing or be willing to obtain within three years.
  • Two years of experience as a clinical nurse, with demonstrated leadership skills.
  • Previous experience and knowledge of Diabetes Management preferred.
  • Current licensure as a Registered Nurse in state(s) of practice and/or possess multi-state licensure privileges as required by position.
  • Certification as a Certified Diabetes Educator from the National Certification Board of Diabetes Educators or obtain certification within three years when eligible.
  • Basic Life Support (BLS) certification is required.

Qualifications

  • Graduate from an accredited nursing program preferred, including AACN, ACEN, and NLN CNEA.

Skills

  • Familiarity with pathophysiology, epidemiology, and clinical practice consistent with practice level.
  • Ability to apply principles of program and/or business management to support successful self-management of diabetes.

Pay

$29.50 - $44.50 per hour

Schedule

Full time, 8 hours per day, Monday - Friday, 8am-5pm, with 7th weekend call rotation at the Medical Center.
