Responsibilities
- Coordinate medical services with each member’s primary care physician or medical clinic and oversee all healthcare services provided to the members at the program, including but not limited to therapies.
- Member of the core professional interdisciplinary team.
- Oversee the indirect and direct nursing care provided to members receiving day habilitation services.
- Administer medications and treatments prescribed by the member’s physician in accordance with approved nursing techniques.
- Supervise or provide direct care and training in relevant day habilitation areas including individual specific medical protocols.
- Educate members families, staff, residential providers and members in hygiene and health concerns.
- Cookordinate each members day habilitation service plan with other health care professionals including the nursing facility where the member resides, if applicable.
- Complete the individual’s medical plan of care as part of the Day Habilitation Service Plan. Ensure individual’s Day Habilitation Service Plan is documented accordingly.
- Ensure nurses progress notes are recorded monthly.
- Maintain updated information relative to each member’s current medications, PRN’s, side effects, diagnosis, annual physical, immunizations, physician’s authorizations, orders and treatments.
- Participate in the Positive Behavior Support initiative and complete the Quality Behavior Curriculum.
- Participate in the continuing core professional interdisciplinary evaluation and review of each member’s needs and status. Document in each member’s record, any recommendation resulting from the periodic review.
- Advise the program director of any medical problems that may hinder progress of members or that may be aggravated by activities at the program.
- Explain procedures and treatments to individuals to gain cooperation, understanding and minimize apprehension.
- Complete Recipient Severity Profile/Clinical Assessment in cooperation with the core professional interdisciplinary team for each member upon admission, every two years and/or with significant change in medical status.
- Participate in quality improvement activities.
- Document all conferences with family members and outside professionals in member’s records.
- Attend all required meetings.
- Adhere to agency safety standards.
- Perform additional duties, or substitute for others as required.
- Adhere to all RTR policies and procedures.
- Develop and provide all services in accordance with RTR’s philosophies including staff and Board beliefs.
Qualifications
- Must be licensed as a registered nurse with the Commonwealth of Massachusetts.
- Have a least one year of relevant experience.
- Valid Driver’s License.
- Basic knowledge of Microsoft Word and the internet.
- Successful completion of all state and agency mandated trainings.
- Must be willing to authorize a Criminal History Background Check (CORI) and release of information concerning allegations from Disabled Persons Protection Commission (DPPC), Department of Developmental Services (DDS), and other agencies.
- Must be able to perform each essential duty satisfactorily.

- Tuberculosis screening within the 12 previous months.
Reasoning Ability

The employee is occasionally required to taste or smell. The employee must frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 60 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
