Responsibilities

  • Applies professional nursing judgment to evaluate hospice documentation for clinical appropriateness, regulatory compliance, and eligibility requirements.
  • Conducts a comprehensive clinical review of the initial plan of care and collaborates with clinical team to ensure all orders, goals, and interventions are present.
  • Serves as a subject matter expert in electronic medical records (EMR) and hospice platforms, including SHP.
  • Oversees workflow, including but not limited to admission, discharge, and HOPE.
  • Provides clinical consultation, education, and mentoring to clinical team members regarding best practices and compliance with regulatory requirements.
  • Collaborates effectively with members of the interdisciplinary team to support quality patient care and operational excellence.
  • Utilizes operational data to identify systemic trends, develops recommendations, and partners with leadership to implement process improvements.

Requirements

  • Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
  • Minimum of three to five (5) years’ relevant experience as a registered nurse in a Hospice environment; preferred experience in hospice quality activities.
  • Current State License as a Registered Nurse.
  • Must have a dependable vehicle, a valid driver’s license, and current auto insurance in accordance with the laws of the State.
  • 10% travel.
  • Meets all applicable agency policies and procedures related to health screening and required testing.

You will report to the Director of Patient Access.
