Job Description

As an Elara Caring Clinical Team Manager, you’ll contribute to our success by:

  • Providing oversight of all patient care services and personnel
  • Planning, directing, and participating in the delivery of home health services
  • Ensuring the timely start of patient care within recommended regulatory guidelines
  • Processing and coordinating physician orders, staff assignments, and communications
  • Reviewing and approving plan of care and evaluating proposed changes
  • Conducting regular OASIS reviews for all patients
  • Making regular meetings with direct care team members for guidance and communication
  • Initiating hiring requests and recommending candidates for direct care staff
  • Communicating frequently with the Branch Director or Administrator on key performance metrics
  • Remaining available for field work, as needed

What is Required?

  • Associate degree in a Nursing related field required
  • 2 years home care experience as a registered nurse in a Home Health environment (with OASIS)
  • 1 year of supervisory experience preferred
  • Current, unrestricted RN license valid for the state of work
  • Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office
  • Able and willing to travel within branch/office coverage area
  • Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
