RN - Clinical Team Leader - Home Health
About the role
Enhabit Home Health & Hospice is seeking a professional to coordinate home health services, ensuring delivery according to accepted standards of practice and company procedures. The role involves reviewing and approving patient information, and facilitating relationships among physicians, referral sources, patients, caregivers, and employees.

Responsibilities
- Ensure overall coordination of home health services is delivered according to acceptable standards of practice and company procedures.
- Review and approve patient information submitted by the licensed professional during start of care, recertification, resumption of care, or evaluation visits.
- Facilitate relationships between physicians, referral sources, patients, caregivers, and employees.
Qualifications
- Graduate of an approved school of professional nursing.
- Licensed in the state of practice, or eligible under the nurse licensure compact.
- At least two years of nursing experience.
- Minimum one year of experience in home health or hospice.
- Demonstrated knowledge of federal, state, and local laws and regulatory guidelines governing home care operations.
- Basic technology skills, including operation of a mobile device.
Education and Experience (Preferred)
- Previous experience in management is preferred.
Requirements
- Valid state driver’s license.
- Maintain automobile liability insurance as required by law.
- Dependable transportation in good working condition.
- Ability to safely drive an automobile in all weather conditions.
