Hospice of the Midwest is seeking a passionate, dedicated Hospice Clinical Supervisor to join our team. This role ensures high-quality patient care by overseeing hospice staff, coordinating services, and guiding clinical decisions. The ideal candidate is a strong communicator and leader, with a passion for providing exceptional end-of-life care.
Office location and schedule
Location: 9766 Fallon Avenue Suite 201 Monticello, MN 55362
Schedule: Monday-Friday 8AM-5PM
Responsibilities
- Review new patient referrals and determine care needs; assign appropriate hospice staff
- Support and guide clinical staff through home visits, case reviews, and real-time consultations
- Help develop patient care plans, set treatment goals, and prioritize care
- Lead and participate in team case conferences to ensure coordinated, holistic care
- Conduct regular chart audits and provide feedback for quality improvement
- Participate in hiring, onboarding, and training of clinical team members
- Support staff development and continuing education programs
- Assist in policy development and uphold professional care standards
- Engage in community outreach and promote the hospice's mission and services
- Contribute to quality improvement initiatives and organizational goals
Qualifications
- Registered Nurse with current, unencumbered licensure to practice professional nursing in the state of operation
- Graduate of an accredited school of nursing; BSN preferred
- Previous experience in hospice or home care setting required
- Minimum of two (2) years of management or supervisory experience preferred
- Current CPR certification, maintained in accordance with organizational policy
- Must be a licensed driver with an automobile that is insured per state and/or organizational requirements and is in good working order
- Proven ability to work effectively within an interdisciplinary team setting
- Demonstrates excellent observation, nursing judgment, and communication skills—both verbal and written
- Understands the hospice philosophy and the emotional, clinical, and ethical considerations surrounding end-of-life care
- Knowledge of pain and symptom management principles
- Adheres to accepted professional standards and practices
- Commitment to clinical excellence and thorough documentation
