Hospice of the Midwest is seeking a passionate, dedicated Hospice Clinical Supervisor to join our team. This role ensures high-quality patient care by overseeing hospice staff, coordinating services, and guiding clinical decisions. The ideal candidate is a strong communicator and leader, with a passion for providing exceptional end-of-life care.

Office location and schedule

Location: 9766 Fallon Avenue Suite 201 Monticello, MN 55362
Schedule: Monday-Friday 8AM-5PM

Responsibilities

  • Review new patient referrals and determine care needs; assign appropriate hospice staff
  • Support and guide clinical staff through home visits, case reviews, and real-time consultations
  • Help develop patient care plans, set treatment goals, and prioritize care
  • Lead and participate in team case conferences to ensure coordinated, holistic care
  • Conduct regular chart audits and provide feedback for quality improvement
  • Participate in hiring, onboarding, and training of clinical team members
  • Support staff development and continuing education programs
  • Assist in policy development and uphold professional care standards
  • Engage in community outreach and promote the hospice's mission and services
  • Contribute to quality improvement initiatives and organizational goals

Qualifications

  • Registered Nurse with current, unencumbered licensure to practice professional nursing in the state of operation
  • Graduate of an accredited school of nursing; BSN preferred
  • Previous experience in hospice or home care setting required
  • Minimum of two (2) years of management or supervisory experience preferred
  • Current CPR certification, maintained in accordance with organizational policy
  • Must be a licensed driver with an automobile that is insured per state and/or organizational requirements and is in good working order
  • Proven ability to work effectively within an interdisciplinary team setting
  • Demonstrates excellent observation, nursing judgment, and communication skills—both verbal and written
  • Understands the hospice philosophy and the emotional, clinical, and ethical considerations surrounding end-of-life care
  • Knowledge of pain and symptom management principles
  • Adheres to accepted professional standards and practices
  • Commitment to clinical excellence and thorough documentation
