About the Role
Join our caring community as a Clinical Manager, supporting a team of clinicians delivering high-quality home health care.
Responsibilities
- Guiding, supporting, and educating clinicians in care planning, clinical decision-making, and goal setting.
- Ensuring accurate clinical documentation, audits, and billing compliance with Medicare and payer standards.
- Participating in hiring, training, performance evaluations, and coaching of staff.
- Reviewing clinical documentation for accuracy and compliance, collaborating with field clinicians to resolve issues.
- Assisting with OASIS data validation and supporting quality improvement initiatives.
- Monitoring quality metrics, contributing to regulatory readiness, and providing staff education on compliance standards.
- Providing limited direct patient care in exceptional circumstances.
Requirements
- Licensed Registered Nurse with a minimum of 2 years' home health or hospice experience.
- Management and leadership experience required.
- Proficiency with home health software (HCHB), and knowledge of CMS PDGM preferred.
- Current CPR certification, valid driver’s license, reliable transportation, and OASIS experience preferred.
