RN Clinical Manager Home Health

Responsibilities

• Recommend employment decisions for staff to the director of operations, including hiring, firing, advancement, promotion, or status changes.
• Ensure overall quality of patient care and clinical performance of nurses and aides.
• Organize clinical operations for the care center.
• Review service requests and determine patient eligibility/suitability for home care services.
• Coordinate patient referrals and admissions with intake.
• Oversee scheduling, payor verification, and precertification requirements.
• Review staff documentation to resolve missing, incomplete, or untimely issues.
• Assist clinicians in establishing immediate and long-term patient goals, setting priorities, and developing the Plan of Care (POC).
• Regularly evaluate patient needs and revise the plan of care in collaboration with clinicians.
• Provide direct patient care infrequently during emergencies.
• Participate in the on-call process.
• Assist with staff training to ensure quality care, compliance, and fiscal responsibility.
• Participate in investigating and resolving patient, family, or physician concerns.

Requirements

• Associate or bachelor’s degree in nursing.
• Current RN license specific to the state(s) of assignment.
• One year of clinical RN experience in home health or hospice.
• One year of RN management experience.
• Current CPR certification.

Pay

$80,000–$88,000, depending on location, job-related knowledge, skills, and experience.
