About the role

The Clinical Manager, Home Health Nursing, oversees clinical operations at a local branch, ensuring quality patient care, staff management, regulatory compliance, and documentation accuracy.

Responsibilities

• Review referrals and assign clinicians
• Support care planning and mentor clinical staff through education and performance evaluations
• Coordinate communication among healthcare providers
• Participate in quality improvement initiatives
• Support organizational goals
• Provide limited direct patient care
• Act as Branch Director when needed

Qualifications

• Nursing degree
• Current RN license
• CPR certification
• At least 2 years of home health and management experience
• Knowledge of OASIS and CMS PDGM
• Valid driver's license and reliable transportation

Skills

• HCHB experience (preferred)
• Certification (preferred)
