RN Clinical Liaison

About the Role

Hospice of Orange & Sullivan Counties (HOSC) is seeking an experienced, compassionate Registered Nurse Clinical Liaison to join our Patient Access team. This is an exciting opportunity for an RN who enjoys combining clinical expertise with relationship-building, education, and care coordination to improve access to hospice services throughout our community. As an RN Clinical Liaison, you will serve as a trusted clinical resource for hospitals, healthcare providers, patients, and families while partnering with our interdisciplinary team to facilitate timely, seamless transitions to hospice care.

You'll play a key role in expanding access to compassionate end-of-life care by educating referral sources, conducting hospice evaluations, supporting admissions, and strengthening community partnerships. This is a dynamic hybrid position that combines clinical practice, relationship management, and education.

Responsibilities

• Build and maintain collaborative relationships with physicians, advanced practice providers, hospital case managers, social workers, discharge planners, nursing leadership, and other referral partners.
• Educate healthcare professionals, patients, and families about hospice philosophy, eligibility criteria, levels of care, and available services.
• Identify patients who may benefit from hospice care and help facilitate timely referrals and admissions.
• Conduct hospice informational visits, clinical evaluations, and admissions as needed.
• Serve as a clinical resource for referral coordinators, navigators, admissions staff, and leadership regarding hospice eligibility and complex referrals.
• Partner with Clinical Operations to coordinate smooth transitions of care from hospitals, skilled nursing facilities, physician offices, assisted living communities, and patients' homes.
• Deliver educational programs for referral sources and community partners while supporting strategic outreach and growth initiatives.
• Collaborate with organizational leadership to improve the referral experience and strengthen community partnerships.
• Participate in weekend, holiday, or after-hours admission coverage on a rotating basis, as assigned.

Your work will include time spent:

• Embedded within assigned hospital partners
• Supporting Patient Access operations at HOSC offices
• Visiting physician practices, skilled nursing facilities, assisted living communities, and the Kaplan Family Hospice Residence
• Working remotely for documentation, referral management, education development, and administrative responsibilities

Requirements

• Graduate of an accredited nursing program
• Current New York State Registered Nurse license
• Minimum of two years of nursing experience
• Valid driver's license, reliable transportation, and proof of insurance

Qualifications

Preferred Qualifications:

• Experience in one or more of the following: Hospice or palliative care, Oncology, Home care, Hospital case management, Discharge planning, Clinical leadership
• CHPN certification is preferred or willingness to obtain within one year of hire

• Comfortable working independently while collaborating across interdisciplinary teams
• Knowledgeable about Medicare regulations, hospice eligibility, and levels of care, or eager to further develop that expertise
• Proficient with electronic medical records and Microsoft Office applications
