RN - Clinical Field Reviewer

About the role

Mountain Pacific, established in Montana in 1973, is a nonprofit organization that partners with communities to deliver better health solutions using federal, state, and commercial funding. The RN Clinical Field Reviewer is a remote position requiring in‑home nursing assessments for members of the Medicaid CFCS/PCS program. The successful candidate must reside in one of the following Montana counties: Gallatin, Park, Sweetgrass, Stillwater, Carbon, or Yellowstone.

Responsibilities

• Conduct person‑centered in‑home nursing assessments focused on members’ functional abilities and ADL/IADL needs.
• Perform assessments initially, annually, and as needed when a change in condition occurs.
• Develop care plans, provide care coordination, and empower members to participate in their own care through active listening and planning
