About the role
The Clinical Care Leader (CCL) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care in the home setting. Collaborates with other inter-professional colleagues to plan, implement and evaluate care. Documents resident history, symptoms, medications and care provided in compliance with applicable standards/regulations. Utilizes technology to enhance quality resident care, to collaborate with inter-professional team members, effectively delegate to and supervise unlicensed personnel, assume overall nursing accountability for a resident assignment and serve in leadership roles on department/organizational tasks forces and committees.
Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in the specific care area or unit. Must have effective communication skills to collaborate, negotiate and resolve conflicts, situations, and/or issues with peers, inter-professional colleagues, residents and families. Must have strong clinical reasoning and critical thinking skills to analyze data and appropriately respond to changing situations, including coordination of care, resident assessment, resident education, triage, and various other nursing interventions.
Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.
Responsibilities
- Utilize the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to provide individualized nursing care in the home setting.
- Collaborate with inter-professional colleagues to plan, implement, and evaluate care.
- Document resident history, symptoms, medications, and care provided in compliance with applicable standards/regulations.
- Use technology to enhance quality resident care and collaborate with the inter-professional team.
- Delegate to and supervise unlicensed personnel.
- Assume overall nursing accountability for resident assignments.
- Serve in leadership roles on department/organizational task forces and committees.
- Demonstrate competency and practice within the full scope of nursing expertise/knowledge.
- Utilize age and population-specific standards in care delivery.
- Communicate effectively to collaborate, negotiate, and resolve conflicts.
- Apply clinical reasoning and critical thinking to analyze data and respond to changing situations.
- Coordinate care, perform resident assessments, provide resident education, and conduct triage.
Requirements
- Graduate from an accredited nursing program, including those accredited by CCNE, ACEN, or NLN CNEA.
- Currently hold an unencumbered RN license with the State Board of Nursing where the practice of nursing occurs and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
- Obtain and maintain required department-specific competencies and certifications.
- Basic Life Support (BLS) certification required.
- Depending on location, a valid driver’s license may be required.
Qualifications
- Bachelor’s degree in Nursing strongly preferred.
- Minimum of two years’ clinical experience preferred.
- Resident Assessment Instrument (RAI) process experience preferred, but not required.

Pay
Salary Range: $37.50 - $55.00 per hour
