Responsibilities
- Affirms patients' physical, psychosocial, cultural, and spiritual needs through observation, interviews, and review of medical records and clinical data.
- Develops and implements patient care plans in collaboration with the interdisciplinary team, observing outcomes in accordance with nursing standards.
- Demonstrates competency in therapeutic and diagnostic interventions, including IVs, wound care, medication administration, catheterization, and specimen collection.
- Advocates for and protects the health, safety, and rights of each patient.
- Provides patient and family education based on assessment of learning readiness, needs, and ability, and evaluates the effectiveness of teaching.
- Encourages patient engagement in healthcare through motivational interviewing, self-management protocols, and teach-back methodology.
- Delivers relationship-based care, engaging patients in all aspects of care with respect for cultural differences and diversity.
- Delegates tasks to non-RN personnel in accordance with State Board of Nursing and hospital policy.
- Captures and coordinates group visits for identified high-risk patient populations.
- Aids in the care transition of high-risk patients, including medication reconciliation and coordination of services.
- Contributes to continuous process improvement by actively participating in patient experience and satisfaction initiatives.

Qualifications
- Associate's of Nursing
- Bachelor's of Nursing (preferred)
