About the Role
A specialized nursing position focused on providing diabetes self-management education and care in a hospital setting.
Responsibilities
- Assess, plan, implement, and evaluate patient care for individuals with diabetes.
- Deliver individualized education to patients and families on diabetes management.
- Coordinate case management, including interpreting lab results, managing medication refills, and facilitating referrals.
- Develop and revise care and discharge plans tailored to patient needs.
- Collaborate with multidisciplinary teams to optimize patient outcomes.
- Support the professional growth of staff through mentoring and training.
- Participate in quality improvement initiatives and apply evidence-based practices.
- Manage independent projects to enhance diabetes care programs.
Requirements
- Valid Massachusetts RN license.
- Minimum of 3 years of RN experience.
- BLS certification.
- Certified Diabetes Educator (CDE) certification or willingness to obtain within one year.
- Preferred: BSN degree.
Skills
- Strong communication and clinical judgment.
- Ability to manage complex cases and mentor staff.
