RN - Certified Diabetes Educator

Role: Registered Nurse - Certified Diabetes Educator, providing specialized nursing care and diabetes self-management education in a hospital setting. Responsibilities: Assessing, planning, implementing, and evaluating patient care; delivering individualized education to patients and families; managing case coordination, including lab result interpretation, medication refills, and referrals; developing and revising care and discharge plans; collaborating with multidisciplinary teams; supporting professional growth of staff and participating in quality improvement initiatives. Qualifications & Skills: Valid Massachusetts RN license, minimum 3 years RN experience, BLS certification, and CDE certification or willingness to obtain within a year. Preferred BSN degree. Strong communication skills, clinical judgment, and ability to manage complex cases. Physical ability to perform patient care tasks and exposure to clinical hazards. This role involves independent project management, mentoring, and applying evidence-based practices to enhance patient outcomes.
