RN Case Mgr-Crestwood Medical Center-Case Management-PRN-1st shift

Responsibilities

• Conduct concurrent case management and discharge planning activities.
• Identify and address barriers to discharge.
• Coordinate with patients, families, physicians, and community resources.
• Facilitate referrals to post-acute providers.
• Participate in interdisciplinary rounds.
• Support patient progression, regulatory compliance, and length-of-stay initiatives.
• Document case management interventions.
• Collaborate with the interdisciplinary team to ensure safe, efficient transition of care.
• Maintain scheduling flexibility, including a minimum commitment of one weekend per month.
• Perform other duties as assigned in support of departmental and organizational objectives.
