RN Case Manager- Visiting Hospice Nurse

The role involves providing compassionate care to patients and families in their homes, coordinating with other healthcare providers, and ensuring that patients receive the best possible end-of-life care.

Responsibilities

• Visit patients in their homes to provide medical care and support
• Collaborate with interdisciplinary teams to develop and implement care plans
• Communicate regularly with patients, families, and other healthcare providers
• Document patient care and treatment in accordance with regulatory requirements

Requirements

• Bachelor's degree in Nursing or related field
• Current New Jersey RN license
• Experience in hospice and palliative care preferred

Qualifications

• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Proficient in computer systems and software

Skills

• Excellent clinical judgment and decision-making skills
• Knowledge of hospice and palliative care practices
• Ability to handle sensitive and confidential information
