RN Case Manager Temp 13 Weeks

About the Role

We are seeking a temporary Registered Nurse - Case Manager to join our team for a 13-week assignment. This position is available in both full-time and part-time capacities, with a schedule of Monday through Friday during the day shift.

Vibra Hospital of Fargo, located in Fargo, ND, is a 31-bed Critical Care Hospital specializing in acute care services for patients recovering from serious illnesses or injuries. Our focus includes medically complex conditions such as trauma, infectious diseases, wound healing, cardiovascular disease, stroke, amputation, and ventilator weaning. As a smaller, specialized hospital, we provide a fast-paced and friendly work environment where you can build meaningful relationships with patients and their families.

Responsibilities

• Coordinate clinically complex patient care across a continuum, ensuring and facilitating quality clinical and cost outcomes.
• Negotiate, procure, and coordinate appropriate services and resources needed by patients.
• Intervene at key points to address and resolve issues or concerns.
• Ensure appropriate sequencing of treatment goals and implementation of a comprehensive discharge plan.
• Demonstrate knowledge and skills necessary to provide case management services to patients with rehabilitation needs.

Requirements

• Current, valid, and active license to practice as a Registered Nurse, Occupational Therapist, Physical Therapist, Speech Therapist, or Licensed Social Worker in the state of employment; OR a Master’s degree in Social Work with at least three (3) years of hospital case management experience.
• Current BLS certification from a Vibra-approved vendor.
• One (1) year of case management experience preferred.
• Previous LTAC (Long-Term Acute Care) experience preferred.
• CCM (Certified Case Manager) or ACM (Accredited Case Manager) certification preferred.
• Working knowledge and ability to apply professional standards of practice in job situations.
• Assertive and personable with the ability to balance delivering quality, cost-appropriate, and service-oriented outcomes.
• Comprehensive knowledge of case management principles, concepts, and applicable standards from voluntary and regulatory agencies.
• Proficient in collecting and retrieving material from medical records.
• Knowledgeable in utilization review, medical terminology, appropriate levels of care, treatment modalities, statutes, and healthcare delivery models.
• Outstanding written and oral communication skills, practiced with all internal and external business relationships.
• Highly refined interpersonal relationship abilities, including negotiation and problem-solving skills.
• Strong clinical assessment and process skills.
• Demonstrated critical thinking skills in all situations.
• Ability to project a professional image.
• Knowledge of regulatory standards and compliance requirements.
• Strong organizational, prioritizing, and analytical skills.

Schedule
