RN Case Manager - South Facilities Team

About the role

Empath Health is seeking a RN Case Manager to join our South Facilities Team. Tidewell Hospice, a member of Empath Health, serves families across southwest Florida with compassionate, dignified care.

Responsibilities

• Create and maintain a plan of care from admission to the end of the patient's hospice experience.
• Serve as the coordinator for patient and family interaction with Tidewell Hospice.
• Listen, observe, communicate, and sensitively provide nursing services in emotionally intense situations.
• Review and update the care plan, physician orders, pharmacy profile, and other records.
• Educate patients, family members, and other caregivers about the disease process, decline, available palliative interventions, the dying process, and safety practices.

Requirements

• Graduate of an approved nursing program.
• Own a current Florida RN license.
• Minimum one year experience in an acute-care hospital environment providing direct patient care.
• Previous experience as a hospice RN preferred.
