RN Case Manager - Sign On Bonus

Key Responsibilities

• Provide private duty nursing care to patients in their homes to help with caseload growth until full to continue in a supervisory capacity.
• Provide skilled nursing visits to establish census.
• Administer medication, treatments, and therapies as prescribed by the physician.
• Monitor patient's condition and report any changes to the healthcare team.
• Auxiliary in activities of daily living and provide emotional support to patients and their families.
• Collaborate with other healthcare professionals to ensure optimal patient care.
• Maintain accurate and complete medical records using web-based electronic medical records system.
• Aid in marketing and market growth development in Grand Junction and the surrounding areas.
• Participate in careers fairs, partner with local universities, and attend various marketing opportunities as needed.

Requirements

• Active RN License.
• Previous Home Health experience required.
• Current BLS Certification.
• Valid Driver's License and Vehicle Insurance.
• Strong time management and prioritization skills.
• Excellent communication, assessment skills and attention to detail.

Pay

$87,000-$93,000 annually, with flexibility based on experience and overall fit.
