RN Case Manager PRN
Job Summary
This is a premium pay position requiring availability every other weekend (4 shifts a month) with the possibility of picking up additional shifts as schedule allows. Join Mountain View Hospital, part of HCA Healthcare, as an RN Case Manager PRN.
Qualifications
- Basic Cardiac Life Support within 30 days of employment start date (RN)
- Registered Nurse degree
- Associate Degree preferred
- Previous Hospital Case Management experience strongly preferred
- Two or more years of hospital RN experience required (with critical care experience preferred)

About the Role
Support patients and families through every step of their care journey by coordinating services, connecting resources, and developing care plans that reflect each patient’s unique needs. Partner with physicians, nurses, and department leaders to ensure safe transitions, clear communication, and consistent quality across the continuum of care.
Responsibilities
- Guide patients and families through program orientation, explain the rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights
- Coordinate education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences
- Develop and implement individualized treatment plans that reflect the patient’s strengths, needs, and personal recovery goals
- Complete psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care team
- Collaborate closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care
