RN Case Manager-Private Duty

Traditional Home Health Care, Pediatric & Adult Skilled Home Care Division seeks a Home Health (RN) Case Manager - Full Time due to expansion and significant growth. This salaried position offers an aggressive bonus plan and a generous benefit package in a non‑corporate, family‑owned atmosphere.

Responsibilities

• Receives referrals, determines services required, and Agency’s ability to meet needs for pediatric and waiver patients.
• Conducts supervisory visits to observe clinical skills and ensure exemplary client care and compliance.
• Monitors documentation of pediatric and waiver patient clinical records to ensure compliance with regulatory standards of timeliness, accuracy, and completeness.
• Manages/supervises a team of RNs, LPNs, Home Health Aides to provide effective and quality home care services.

Qualifications

• Prior experience in scheduling/human resources/customer service required.
• Current license as a Registered Nurse in Pennsylvania.
• Demonstrated organizational and time management skills, excellent communication skills, interpersonal skills, documentation skills, critical thinking, and ability to work independently.
• Ambition to grow and advance with company goals.
