RN Case Manager Per Diem

Responsibilities

• Complete an initial assessment of patient and family to determine hospice care needs.
• Regularly re‑evaluate the patient’s nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the entire healthcare team, patient, and family to direct work that provides continuous, well‑coordinated care.
• Counsel the patient and family in meeting end‑of‑life needs.
• Initiate appropriate preventative and palliative nursing procedures.
• Communicate with the physician regarding the patient’s needs and report any changes in condition.
• Supervise the hospice aides.

Qualifications

• Associate’s Degree in Nursing.
• Current State Certified RN license.
• Minimum 2 years of professional nursing experience (hospice experience preferred).
• Valid driver’s license and reliable transportation.
• Willingness to travel as needed.
• Strong verbal and written communication skills.
