RN Case Manager (Part Time/Per Diem)

Position Highlights

Location: El Paso, TX – hybrid/remote field‑based role with a dedicated home office. Travel within an assigned territory covering El Paso and surrounding areas, with occasional remote coverage in other regions. Territory assignments may change based on business needs.

Schedule: Monday – Friday with flexibility to accommodate patient needs and nephrology partner availability.

Employment Type: Per Diem.

This autonomous role collaborates with the entire healthcare team to coordinate and facilitate quality, cost‑effective care for ESRD and CKD patients, linking patients, families, physicians, providers, care teams, the community, and the Regional Operations Manager.

DaVita Integrated Kidney Care (DaVita IKC) is the integrated care division of DaVita Inc., delivering care to more than 20,000 patients with late‑stage CKD and ESRD across the U.S. each month, focused on improving patient quality of life, outcomes, and total cost of care.

Qualifications

• A.D.N. degree from an accredited school of nursing (required); B.S.N. preferred; a three‑year diploma from an accredited program may substitute for a nursing degree.
• Current RN license in the state of practice.
• Continuing education credits as required by state of practice.
• Minimum five (5) years of clinical nursing experience (strongly preferred).
• Minimum three (3) years of renal nursing experience (preferred).
• Case management experience (required). If not, at least three (3) years of experience assessing patient needs, developing and managing individualized care plans, coordinating interdisciplinary services, and supporting patients through complex healthcare transitions.
• Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred.
• Demonstrated knowledge of data management to achieve clinical, financial, and patient‑satisfaction outcomes.
• Experience acting as a change agent and applying CQI techniques.
• Previous healthcare performance‑coaching experience.
• Current CPR certification.
• Ability to adapt practice patterns to new electronic processes and productivity expectations (Capella implementation).
• Valid driver’s license with a positive driving record and ability to meet insurance requirements.
• Basic computer skills; proficiency in MS Word and Outlook.
• Functional proficiency with DaVita clinical software (including Capella) within 90 days of employment.
• Home office with internet connectivity of at least 1 MB upload and 1 MB download.
