Overview

Missouri Baptist Sullivan Hospital is seeking a compassionate, organized, and collaborative RN Case Manager to join their team. The RN Case Manager plays a vital role in coordinating patient care, facilitating safe and timely discharges, reducing avoidable admissions, and connecting patients with appropriate post-acute resources.

Responsibilities

• Assesses, coordinates, and evaluates each patient's plan of care.
• Establishes, continuously evaluates, and documents an individualized discharge plan.
• Ensures clinical documentation and cost-effective use of hospital resources.

Requirements

• Minimum Requirements:
  • Education: Nursing Diploma/Associate's Nursing
  • Experience: No Experience
  • Licenses & Certifications: RN
• Preferred Requirements:
  • Education: Bachelor's Degree Nursing
  • Experience: 2-5 years

Benefits

Additional Preferred Requirements include 2 years of clinical RN experience and/or previous case management experience, BSN, Ability to work collaboratively in a fast-paced environment, Prior experience with EPIC.
