About the role

The Oregon Health Authority (OHA) seeks an RN Case Manager (Mental Health Registered Nurse) at the Oregon State Hospital, Salem Campus. This role is responsible for identifying and monitoring patients with chronic medical conditions or those included in assigned clinical metrics.

Responsibilities

  • Conduct comprehensive and ongoing patient assessments through review of lab results, medication adherence, observation, physical examination, and patient interviews.
  • Determine case closure by evaluating patient needs, collaborating with OSH staff, and coordinating with receiving entities.
  • Obtain medication authorizations, support medication management, and act as a liaison between pharmacy and physicians.
  • Facilitate referrals, provide follow-up services, and promote quality, cost-effective patient care while adhering to professional case management standards.
  • Facilitate interdisciplinary meetings, coordinate admissions, transfers, and discharges, and assist social work with establishing community providers to ensure continuity of care for complex patients.

Requirements

  • Possess a valid Oregon Professional Registered Nurse License at the time of appointment.
  • A Bachelor of Science in Nursing plus 2 years of nursing experience providing medically focused patient care, OR an Associate Degree in Nursing plus 5 years of nursing experience; 2 of those years providing medically focused patient care.

Qualifications

  • Skilled in coordinating care, determining case closure, collaborating with multidisciplinary teams, and following case management standards and protocols.
  • Effective in working with physicians, pharmacy staff, social work, and external providers to ensure continuity of care.
  • Comfortable participating in team meetings (IDT, PET) and working across departments to support admissions, transfers, and discharges.
  • Good understanding of medication authorization processes, collaboration with pharmacy, and use of treatment algorithms as guided by providers.

Schedule

This is an on-site position.

Pay

The salary listed is the non-PERS qualifying range. If the successful candidate qualifies for PERS, the salary range will increase by 6.95%.
