RN Case Manager - Hospice - Days

Join a dedicated hospice nursing team responsible for delivering personalized patient care plans that promote physical, psychosocial, emotional, and spiritual well-being. Responsibilities Include Assuming full nursing responsibility for patient care and continuously evaluating performance to improve quality and productivity.Adhering to organizational policies, procedures, and goals to ensure high-quality care.Making independent, sound clinical decisions in a cost-effective manner without compromising quality.Developing and implementing patient care strategies in collaboration with healthcare professionals.Maintaining patient confidentiality and demonstrating effective verbal and written communication skills.Understanding the needs of patients and families facing end-of-life issues with compassion and sensitivity. Qualifications Include Graduate from an accredited nursing school.At least one year of clinical experience and hospice care experience.Current Georgia RN licensure or multistate license.CPR certification and valid driver’s license with dependable transportation.
