Job Summary

A hospice RN case manager plays a crucial role in providing compassionate care to patients with terminal illnesses, coordinating their treatment plans, and supporting their families throughout the end-of-life journey.

Requirements

  • RN Registered Nurse in Texas or Compact state in good standing on license
  • A valid RN license is mandatory, along with certifications in hospice and palliative care being advantageous
  • Experience with RN Case Management in Hospice of at least one (1) year
  • Prior experience in nursing, particularly in hospice or palliative care settings is needed
  • Skills in crisis management and interdisciplinary teamwork are essential

Skills

  • Typically, a hospice RN case manager holds a Bachelor’s or Master’s Degree in Nursing, with relevant coursework in palliative care and healthcare management
  • Experience with HCHB EMR
