Responsibilities
- Conduct initial and ongoing patient assessments.
- Develop and manage individualized hospice care plans.
- Administer medications and provide palliative interventions.
- Educate patients and families on symptom management and care needs.
- Provide emotional support to patients and families.
- Manage and coordinate care for assigned patients.
- Collaborate with physicians and interdisciplinary team members.
- Participate in care conferences and family meetings.
- Advocate for patient needs and ensure access to resources.
- Maintain accurate and timely clinical documentation.
- Supervise and support hospice aides.
- Ensure necessary equipment and supplies are in place.
- Participate in on-call rotation (5 days/month).
- Contribute to quality improvement initiatives.
Requirements
- Active RN license in Minnesota.
- Graduate of an accredited School of Nursing.
- CPR certification.
- 1+ year of acute care experience required.
- Hospice or home care experience preferred.
- Strong assessment and clinical decision-making skills.
- Valid driver's license and insured vehicle.
Schedule
Full-time, Monday–Friday with on-call rotation (5 days/month).
