RN Case Manager - Hospice

Role Overview Assumes full nursing responsibility for delivering personalized hospice patient care, developing and implementing comprehensive care plans in collaboration with healthcare professionals. Maintains high-quality standards by evaluating performance, adhering to policies, and ensuring cost-effective, sound decision-making without compromising care. Provides holistic treatment strategies that support physical, psychosocial, emotional, and spiritual well-being, with a focus on patients facing end-of-life challenges. Effective communication and confidentiality are essential, along with understanding the unique needs of patients and families confronting death and dying. Qualifications Graduate from an accredited nursing school.At least 1 year of clinical experience; hospice experience preferred.Valid Georgia RN licensure or multistate licensure.Current CPR certification.Valid driver’s license, insurance, dependable transportation or access to public transit.
