RN Case Manager - Hospice

We are seeking a dedicated RN Case Manager to coordinate and deliver compassionate hospice care. Responsibilities include: Developing and overseeing individualized care plans with patients, families, and the interdisciplinary team; providing skilled nursing assessments, interventions, and supportive care; supervising Certified Nurse Aides; coordinating patient care with physicians and team members; ensuring educational needs are met; performing complex triages and evaluating patient transfer needs; and participating in community education and team training. Qualifications and skills: Graduate of an accredited nursing program (BSN preferred); 1-2 years of hospice, home health, or related clinical experience; current Colorado RN license; valid driver’s license and auto insurance; CPR certification; ability to lift at least 35 lbs; and willingness to travel daily.
