RN Case Manager - Home Hospice (Full-Time Day)

Role: Hospice RN Case Manager providing full-time home hospice care with daily travel to patients' homes. Responsibilities: Assessing, planning, implementing, and evaluating patient care; managing symptoms and medications; educating patients and families on disease progression and end-of-life care; coordinating interdisciplinary team efforts; preparing patients and families for death; documenting care and maintaining compliance with regulations. Qualifications & Skills: RN license (single or multi-state), BLS certification, valid driver's license, and private vehicle insurance. Preferred certifications include Hospice Palliative Nurse (CHPN). Minimum two years of hospice, home care, or acute care experience. Essential skills include emotional maturity, strong assessment and clinical judgment, excellent communication, and computer proficiency. Additional Duties: Supervise and collaborate with staff, participate in performance improvement activities, and serve as a resource for clinical staff. Requirements: Clinical excellence in hospice or related fields preferred; ability to provide compassionate, patient-centered care; adherence to policies and standards.
