About the Role

This position covers multiple territories spanning Northwest Arkansas and Northeast Oklahoma. The RN Case Manager assumes primary responsibility for assigned patients and performs all phases of the nursing process in a hospice setting.

Responsibilities

  • Complete comprehensive physical assessments, pain assessments, and symptom control management related to terminal illness.
  • Coordinate Circle of Life services for patients and families.
  • Complete required documentation (assessments, care plans, clinical notes, agency tracking forms) in a timely manner per COL policies.
  • Establish therapeutic nursing relationships with hospice patients, families, and caregivers.
  • Provide weekend coverage and assist during times of excessive nursing absenteeism.
  • Communicate with attending physicians, staff, and other agencies to coordinate optimal care and resource use.
  • Educate patients and caregivers on hospice philosophy, disease progression, physical care components, stages of dying, and other aspects of the Plan of Care (POC).
  • Document all patient-related information according to policies and procedures.
  • Seek input from Interdisciplinary Team (IDT) members regarding the patient POC for additional knowledge and support.
  • Review and update the POC at least every 60 days per regulatory standards.
  • Regularly attend and participate in IDT meetings to ensure the POC reflects current patient care needs.
  • Prepare in advance for IDT meetings.
  • Assess the need for Certified Nursing Assistant (CNA) services and conduct supervisory visits per regulatory standards.
  • Revise the CNA care plan as needed.
  • Inform the Primary Care Manager (PCM) of unusual or potentially problematic patient/caregiver issues.
  • Maintain annual clinical competencies.
  • Model prompt and regular attendance.
  • Exhibit cost-effective use of time, mileage, and supplies.
  • Demonstrate familiarity with and adherence to policies, procedures, state, federal, and accrediting agency requirements.
  • Provide notification of absence per policy.
  • Promote and maintain respect for rights, privacy, and confidentiality of others.

Requirements

  • Current unrestricted Arkansas RN license.
  • BLS certification.
  • Strong assessment skills with critical thinking and problem-solving abilities in a healthcare setting.
  • Strong documentation and communication skills, with the ability to stay calm during emergencies and human suffering.
  • Preferred: Hospice experience, home health, or acute care nursing experience with computer charting.
