RN Case Manager-Home Health (South)

Job Responsibilities

  • Provides direct patient care and case management.
  • Competently implements 24-hour responsibility for each patient including nursing and home health aide service.
  • Identifies and documents patient and family needs through comprehensive assessment.
  • Reports all findings to M.D., coordinates services, and follows through on Physician orders.
  • Performs admission procedure as per RVNAhealth policies using legible, accurate documentation.
  • Develops a plan of care consistent with the physician's plan of treatment.
  • Performs evaluation of patients' progress, promptly acts on any changes made to patient's plan of treatment or condition, including appropriate discharge procedure when goals have been met.
  • Updates and maintains the nursing care plan.
  • Documents communication and interprets plan of care to the patient regarding diagnosis and treatment.

Qualifications/Expectations

  • Graduation from an accredited School of Nursing.
  • Currently licensed to practice as an RN in the State of Connecticut or Compact License required.
  • Has a minimum of one year of nursing experience.
  • Able and willing to follow Agency policies and procedures and to work as a team member supporting the agency mission, vision, and values.
