RN Case Manager – Home Health

What You'll Do

• Build and guide care plans that keep clients safe and supported at home
• Partner with families, physicians, and care teams to keep everyone aligned
• Support and coach field staff so they feel confident and cared for
• Spot problems early and help solve them before they become crises
• Help ensure care stays consistent, covered, and well-coordinated over time
• Keep documentation clear and timely—so care is protected and benefits aren't delayed

Requirements

• Active RN license
• A steady, compassionate leadership style
• Strong communication and follow-through
• Comfort guiding others and building trust with families
• Home health experience is a plus (but not required for the right nurse)
