RN Case Manager - Home Health Agency - PRN or Contractor / As Needed

About the role

Provide comprehensive care coordination for patients in home health settings. Manage patient care plans, coordinate services, and ensure compliance with regulatory standards.

Responsibilities

• Assess patient needs and develop care plans
• Coordinate services with other healthcare providers
• Monitor patient progress and adjust care plans as necessary
• Ensure compliance with Medicare and Medicaid regulations
• Communicate effectively with patients, families, and interdisciplinary team members

Requirements

• Bachelor’s degree in Nursing or related field
• Current RN license in good standing
• Experience in home health care or related field
• Strong organizational and communication skills

Qualifications

• Excellent interpersonal skills
• Proficient in Microsoft Office applications
• Ability to work flexible hours including evenings and weekends
