RN Case Manager- Home care assessment only

About The Role

ComForCare Home Health Care - Florida Tampa/NW Tampa is seeking a dedicated and compassionate RN Case Manager to join their growing team. This assessment-focused role involves making a meaningful difference in patients' lives by conducting in-home evaluations and ensuring they receive the highest quality of care.

Responsibilities

• Conduct comprehensive in-home patient assessments to determine care needs and eligibility
• Develop and document individualized care plans based on assessment findings
• Evaluate patient health status, functional abilities, and home environment safety
• Collaborate with physicians, care teams, and family members to coordinate appropriate services
• Ensure accurate and timely clinical documentation in compliance with regulatory standards
• Monitor and reassess patient conditions, adjusting care plans as needed
• Serve as a patient advocate, connecting clients with community resources and support services

Requirements

• Active and unrestricted Registered Nurse (RN) license in the state of Florida
• Prior experience in home health, case management, or community health nursing preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills with patients and families
• Ability to work independently and manage a flexible schedule in the field
• Reliable transportation and valid driver's license required
• Proficiency with electronic health records (EHR) and clinical documentation

Compensation

$30.00 - $50.00 per hour
