RN Case Manager - Full-Time

About the role

We are seeking a qualified Registered Nurse to provide quality healthcare outcomes through safe, individualized discharge planning and proficient utilization management services.

Responsibilities

• Collaborating with patients, families, and healthcare teams to ensure timely and effective discharge processes
• Maximizing reimbursement
• Meeting clinical and social needs post-discharge

Qualifications

• Graduate of an accredited nursing school
• Current Nevada RN license
• Minimum three years of clinical experience
• Two years in Utilization Review, Management, or Case Management preferred

Skills

• Proficiency in computer programs for word processing, data collection, and management
• Knowledge of social and physical factors affecting discharge and community resources
