RN Case Manager - Barnes-Jewish Hospital (Friday - Monday)
Overview

Responsibilities
- Affords each patient a coordinated, continuous, and comprehensive plan of care.
- Works collaboratively with physicians and interdisciplinary staff, internal and external to the organization, to identify and resolve issues.
- Coordinates, negotiates, procures and manages the care of patients to facilitate achievement of quality cost-effective outcomes.
- Performs and provides consultation regarding Utilization Management and Performance Improvement programs in order to obtain optimum value for the patient and the reimbursement source.
- Assesses, coordinates, and evaluates each patient's plan of care.
- Establishes, continuously evaluates, and documents an individualized discharge plan.
- Ensures clinical documentation and cost-effective use of hospital resources.
Requirements
- Minimum Requirements: Education - Nursing Diploma/Associate's; Experience - No Experience; Licenses & Certifications - RN.
- Prior to employment, candidates must meet the minimum requirements listed above and successfully complete orientation.
Qualifications
- Preferred Qualifications: Education - Bachelor's Degree; Experience - 2-5 years.

Collaborates effectively with interdisciplinary teams, negotiates, and manages patient care plans, performs utilization management and performance improvement programs, assesses and evaluates patient plans of care, establishes and documents individualized discharge plans, ensures clinical documentation and cost-effective use of hospital resources.
