About the role
Transitions At Home, is currently seeking to hire a compassionate and experienced Full-Time RN Case Manager to join our growing home health team. This position offers the opportunity to provide personalized care in a home setting, ensuring clients receive the highest quality of service and support throughout their recovery journey.
Responsibilities
- Provide skilled nursing care to include assessments, medications, treatments, and coordinates services
- Counsels the client and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager
Requirements
- Active Wisconsin Registered Nurse License
- Two (2) years’ nursing experience, 1 year in Home Health (preferred)
- CPR Certification
- A valid Wisconsin Driver's License and Car Insurance

Pay
- $38- $44 per hour starting wage, depending on experience
Schedule
- Full-Time 40 hours per week
- 8:00 am - 4:30pm Monday through Friday
- Limited on-call rotation
