RN Case Manager
Position Overview
Transitions At Home is a trusted provider of home health services, committed to delivering high-quality, compassionate care to patients in the comfort of their homes. We are seeking an experienced RN Case Manager to support our care team and play a vital role in coordinating and delivering patient-centered care.
What Does a Transitions at Home RN Case Manager Do?
This role involves providing expert guidance and oversight in clinical and regulatory matters, staff training, critical thinking, care plan review, incident investigation, and coordinating services. The RN Case Manager also counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
Hours
Full-Time: 40 hours per week, 8:00 AM - 4:30 PM, Monday through Friday. Limited on-call rotation.

Qualifications / Job Requirements
- Active Wisconsin Registered Nurse License
- Two (2) years’ nursing experience, 1 year in Home Health (preferred)
- CPR Certification
- Wound Care would be beneficial
- A valid Wisconsin Driver's License and Car Insurance
