About the role
This role is for an experienced Level 1 RN Case Manager who will be joining a healthcare team in Montana. The position is based in an ambulatory health care setting and involves working as an essential member of a collaborative care team that may include providers, licensed practical nurses, medical assistants, and patient panels.
Responsibilities
- Assess patient needs and develop care plans
- Coordinate care between patients, providers, and other healthcare professionals
- Monitor patient progress and adjust care plans as necessary
- Communicate with patients and families to ensure they understand their care plan
- Ensure compliance with regulatory standards and policies
Requirements
- Bachelor’s degree in Nursing (BSN)
- Current licensure as a Registered Nurse (RN) in the state of Oklahoma
- At least 2 years of relevant clinical experience in a healthcare setting
- Experience in case management or a related field
Qualifications
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in Microsoft Office Suite
Skills
- Excellent organizational and time management skills
- Knowledge of electronic health records (EHR) systems
- Ability to prioritize tasks and manage multiple projects simultaneously
Pay
The pay for this role is commensurate with experience.
